SOCIAL SECURITY ADMINISTRATION

If you need to report a death or apply for benefits, call 1-800-772-1213 (TTY 1-800-325-0778). You can speak to a Social Security representative between 8:00 am – 5:30 pm. Monday through Friday. You can also visit your local Social Security office. An appointment is not required, but if you call ahead and schedule one, it may reduce the time you spend waiting to speak to someone.

VETERANS ADMINISTRATION

Burial benefits available include a gravesite in any of our 142 national cemeteries with available space, opening, and closing of the grave, perpetual care, a Government headstone or marker, a burial flag, and a Presidential Memorial Certificate, at no cost to the family. Some Veterans may also be eligible for Burial Allowances. Cremated remains are buried or inurned in national cemeteries in the same manner and with the same honors as casketed remains.

BURIAL AND PLOT INTERMENT ALLOWANCE

VA burial allowances are flat-rate monetary benefits. They help cover eligible Veterans’ burial and funeral costs. Generally, they are paid at the maximum amount allowed by law.

DEATH CERTIFICATE (COOK COUNTY)

The Cook County Clerk’s office keeps official records of deaths that occur in Chicago and suburban Cook County.

Under state law, you may legally obtain a copy of a death certificate if you are a relative or can prove a financial interest in an individual’s death.

DEATH CERTIFICATE (DUPAGE COUNTY)

You can obtain a certified copy of a death certificate if you are a relative or a person who can prove a financial interest.

NATIONAL ALLIANCE
FOR GRIEVING CHILDREN

The National Alliance for Grieving Children (NAGC) is a nonprofit organization that raises awareness about the needs of children and teens who are grieving a death and provides education and resources for anyone who supports them. Through the collective voice of our members and partners, we educate, advocate, and raise awareness about childhood bereavement.

CENTER FOR LOSS AND TRANSITION

Led by grief counselor and educator Dr. Alan Wolfelt, we are an organization dedicated to helping people who are grieving and those who care for them. Perhaps someone you love has died, or you want to help a grieving friend or family member. Or maybe you are a professional or volunteer grief caregiver. Either way, we are here to offer resources and understanding.

ILLINOIS ATTORNEY GENERAL-CRIME VICTIMS COMPENSATION

The Crime Victim Compensation Act was established by the Illinois General Assembly in 1973 with the primary goal of helping to reduce the financial burden imposed on victims of violent crime and their families. The Illinois Crime Victim Compensation Program can provide eligible victims and their families with up to $27,000 in financial assistance for expenses accrued as a result of a violent crime.
